The personal information collected on this form is collected by DCM (“we”, “us”) in accordance with the Privacy Act 2020. It is collected for the purposes of recording persons entering and leaving these premises, managing health and safety on site, and accounting for all persons in the event of an emergency, in accordance with our obligations under the Health and Safety at Work Act 2015.
Provision of this information is voluntary; however, if it is not provided, we may be unable to permit you entry to the premises. The information will be held securely by us, will be used and disclosed only for the purposes set out above or as otherwise required or permitted by law (including disclosure to emergency services or WorkSafe New Zealand), and will be retained for no longer than 12 months, after which it will be destroyed.
Under Information Privacy Principles 6 and 7 you have the right to request access to, and correction of, the personal information we hold about you.
